While playing a game, hold L + R + X and press Down, Up, Down, Left, Right, Left, A, Up, Left, Down, Right, Up, Right, Down, Left, A, Down(3), A(2). If you entered the code correctly, a message will appear.

Have a saved game file from The Suffering on your memory card or hard drive. When starting a new game you will be able to select your morality depending on how you completed the original The Suffering (good, evil, or neutral).

• Flashy Kill

To instantly kill a burrower, wait for the burrower to surface then simply throw a flash grenade into the hole it came from.

• Full blood

Hold L + R + X and press Up, Left, Down, Right, Up, Right, Down, Left, A during game play. A message will confirm correct code entry.

• Full insanity

• Full xombium bottle

• Gonzo Gun

While playing a game, hold L + R + X and press Left, A(3), Right, Left, Right, Left, Up, A(3), Down, Up, Down, Up, A. If you entered the code correctly, a message will appear.

• Grenades

• Increase good karma

• Old movie mode

Hold L + R + X and press Up, A, Left, A, Down, A, Right, A during game play. A message will confirm correct code entry. Press Start to disable this effect.

• Psychedelic mode

Hold L + R + X and press Left(2), A, Right(2), A, Up(2), A, Down(2), A during game play. A message will confirm correct code entry. Repeat this code to disable its effect.

• Refill ranged weapon ammunition

While playing a game, hold L + R + X and press Left(2), Up, Down, Right, Left, Right(2), A. If you entered the code correctly, a message will appear.

• Reload ammunition for current gun

While playing a game, hold L + R + X and press Right(2), Down, Up, Left, Right, Left(2), A. If you entered the code correctly, a message will appear.

• Rescuing the police officer

At one point in the game, you will encounter a police officer who will ask you to free him. He will also tell you not to turn the three valves in front of you or he will die. If you want to save him, you must first have a full insanity bar. You can get to that state by going back to the previous room and killing the respawning enemies.
Once you have a full insanity bar, open the door and go into the room with the three lockers on the wall. Go into the room after that. Look around you should see a broken wall. Transform into your insane form and carefully destroy the broken wall, making sure you do not kill the police officer. Transform back, and when the police officer runs out and sits down, your good deed will be done. To progress into the level, turn the three valves and stand back. When they explode the previously inaccessible room to the left of the valves will now be open.

• Shotgun with full ammunition

While playing a game, hold L + R + X and press Left(3), Down(3). If you entered the code correctly, a message will appear.

• Suicide

Hold L + R + X and press Down(4) during game play. A message will confirm correct code entry.
