Hold the RT button to kill yourself repeatedly until the "Coward's Way Out" option appears. During the first chapter, hold one of the fat scientists and stand still. Complete the indicated task to unlock the corresponding gamerpic.

To unlock hardcore difficulty, simply beat the game on normal difficulty. You can then play through the game again on hardcore, which requires you to beat each level without dying and any damage results in instant death. If you kill yourself numerous times by holding the Right Trigger, you'll unlock "Coward's Way Out"; a Level Skip option. Splosion Man premium dashboard theme
